What Are Dermal Fillers and How Do They Work?
What is actually being injected, what it does to facial structure, and why the assessment beforehand matters more than the syringe.
Dermal fillers are gel-based products, usually made from manufactured hyaluronic acid, injected beneath the skin to fill out lines and reshape the face. They restore volume where creases and shadows have formed, and can lift and restructure facial contour. The body breaks the product down over time, and it can be dissolved if problems arise. What volume loss does to the shape of a face
A youthful face is characterised by smooth, round contours, high cheekbones, hollow jowls and a thin, well-defined jawline — features that together form what is often called the triangle of beauty, with its base at the top and its point below.
Age inverts that triangle. Facial fat is lost, gravity pulls, and the skin’s natural elasticity goes, so the forehead and temples narrow while the lower face and jawline widen and grow heavier. This is why filling a single line so rarely satisfies anyone. The crease you notice is usually the consequence of volume that has moved or gone, so filler is planned across the face, not crease by crease — nose-to-mouth lines and sagging along the jawline are the clearest examples.

What the treatment feels like
The treatment area is cleaned with an antiseptic and anaesthetic cream is applied before any injection. Several injections may be needed, depending on the area and the plan. Ice packs and skin vibration are both used at Faciem to reduce discomfort, and patients do not generally need local anaesthetic injections.
Afterwards an ice pack may be applied to settle swelling, and the area is massaged so the product distributes evenly. Most people find dermal filler treatment very tolerable, and any discomfort usually settles quickly. The appointment runs 45–60 minutes, much of it assessment and planning, and the change is visible straight away. You see it before you leave, though slight swelling means the settled result looks a little different a few days later.
The products used, and why the range matters
Faciem uses dermal fillers from Allergan, in the Juvéderm® range, using Vycross technology. Vycross stabilises the bonds between hyaluronic acid chains, which makes the product last longer. Within that range, VOLUMA®, VOLIFT and VOLUX® are used to restore facial balance and heart-shaped proportions, revolumising hollows and recontouring the cheeks, cheekbones and nose. The volumising product in particular helps recover facial volume diminished by age or weight loss.
The range exists because different parts of the face need gels of different firmness: what holds structure along a jawline does not belong in a lip. What hyaluronic acid actually is
Hyaluronic acid is a substance that occurs naturally in the body and helps hydrate the skin and give it volume. Your own hyaluronic acid is broken down within a day or two and constantly replaced; with age that replacement mechanism diminishes, so the moisturising and plumping effect declines.
The hyaluronic acid used in filler is stabilised and cross-linked so it lasts for months instead of disappearing within days. Because it is a version of something the body already recognises, it is broken down naturally over time, and it can be dissolved if there are any problems. That reversibility is one reason hyaluronic acid became the standard material for facial work, and it is a fair question to put to any clinic proposing something else.

Filler is planned once and injected in stages, so the sequence below matters as much as the product. One to three sessions are usually required, and the whole plan is agreed at the start.

How Safe Are Dermal Fillers? An Honest Answer
Dermal fillers are a medical treatment, and how safe yours is depends heavily on who assesses and injects you, and on what they know about your history. Careful injection technique minimises bruising and swelling, but it is the consultation beforehand — your medical history, your allergies, what has already been injected — that does most of the work.
Common, temporary effects

Uncommon effects
- These effects do not last and can generally be covered with your usual make-up. - Contraindications apply to this type of treatment generally, not to one product. Before and After Your Dermal Filler Treatment
Preparation matters as much as aftercare with filler, because the main avoidable problem is bruising and most of what reduces it happens before you arrive. The arc afterwards is short: swelling and any bruising are at their most obvious on days one and two, settle over roughly three to five days, and the contour reads true at about two weeks. You are given personalised advice on the day; these are the general principles.
Do
- Tell us the date of every previous aesthetic treatment, including anything injected in the past two years
- Tell us about any blood-thinning medicine at least a week before treatment, so the plan accounts for it
- Use a cool pack for 10 minutes at a time over the first 24 hours if the area feels swollen
- Sleep on your back with your head slightly raised for the first two nights
- Ring the clinic the same day if the area blanches, whitens or becomes more painful after 48 hours
Avoid
- Aspirin and anti-inflammatory painkillers for three days before treatment, where it is medically safe to stop them
- Alcohol for 24 hours before and 24 hours after treatment, as it thins the blood and makes bruising more likely
- Pressing, rubbing or manipulating the treated area yourself for 48 hours after injection
- Saunas, steam rooms, hot yoga and strenuous exercise for 48 hours, as heat and blood flow worsen swelling
- Facials, skin treatments and non-urgent dental work over the treated area for two weeks afterwards

Can I wear make-up after dermal fillers?
Leave the injection points free of make-up for 12 hours after dermal filler treatment, then wear it as normal. The entry points are tiny but still closing during that first half-day, and clean skin keeps the small infection risk smaller. After 12 hours, mineral make-up applied with a clean brush covers redness or a bruise well — pat it on rather than rubbing, and keep firm pressure off the treated area for the first 48 hours.
Can I exercise after dermal fillers?
Leave strenuous exercise for 48 hours after dermal filler treatment. Raised blood pressure, heat and a flushed face all make swelling and bruising more pronounced, and lying face down on a mat or bench presses on product that has not yet settled. Gentle walking on the day is fine. Saunas, steam rooms and hot yoga wait the same 48 hours. If you train most days, book an evening appointment so the rest falls overnight.
Can men have dermal fillers?
Yes. Dermal fillers are planned around the individual face, and male and female facial proportions differ in ways that change where volume should and should not go. A plan that ignores that is how results start looking generic. Where volume is added, and how much, is discussed and agreed at consultation before any treatment.
