Wrinkles: Dynamic and Static Lines — and What Each Needs
Wrinkles are creases that form where facial muscles fold skin whose collagen and elastin are in decline. Dynamic wrinkles appear only while a muscle is working. Static wrinkles are still visible when the face is completely at rest. Most people carry both, and because the two answer to different treatments, the first job at Faciem is working out which of your lines is which.

TL;DR
Wrinkles form where repeated muscle movement folds skin that is losing collagen and elastin. A line seen only during an expression is dynamic; a line still there when the face relaxes is static, and most static lines started out as dynamic ones years earlier. Ultraviolet light is the largest external cause, with smoking, air pollution and the hormonal changes of menopause adding to it. Deep folds beside the mouth are a volume problem, not a crease, so they need a different treatment again. At Faciem, lines are assessed by Dr Prasad, a GMC-registered doctor, before anything is treated.
- Dynamic lines show only on movement
- Ultraviolet light is the largest external cause
- Deep folds are lost volume, not creasing
About the Condition
What Are Wrinkles?
Wrinkles are lines and creases that form where skin is folded in the same place over and over while the collagen and elastin supporting it decline. Dynamic wrinkles appear when a facial muscle contracts and disappear when it relaxes. Static wrinkles remain visible at rest, and most of them began as dynamic lines years earlier.
Dynamic and static wrinkles: how to tell them apart
The distinction that decides your treatment needs nothing but a mirror. Hold the face completely still, then frown, lift the brows and smile. Lines that show up only during an expression are dynamic, folded into the skin by the muscle underneath it. Lines that stay put once everything relaxes are static, held now by the structure of the skin itself. Frown lines between the brows, horizontal forehead lines and the fan at the outer corner of the eye usually arrive first, because those muscles work hardest. Fine crepey lines across the cheek behave differently again, coming from thinning and sun damage instead of expression. Most faces carry a mixture, and a single line can be part dynamic and part static, which is why each area is judged on its own.
Who develops wrinkles, and how early
Dynamic lines turn up in the late twenties and thirties for most people, sooner in anyone with strong expressions or a habit of squinting. Static lines generally follow from the forties onwards. Sun exposure moves that whole timetable forward: outdoor workers and long-term sunbed users often show lines a decade before their contemporaries, and smoking does something similar, especially around the mouth. Family history sets skin thickness and how fast collagen is lost. Deeper skin tones carry more melanin and a thicker dermis, so lines tend to arrive later, while pigment changes appear earlier and are frequently the bigger concern. Skin tone is recorded on the Fitzpatrick scale at assessment, because it changes how the resurfacing side of wrinkle treatment is approached: Fitzpatrick IV to VI skin responds to peels and to laser when the agent, the strength and the interval between sessions are chosen for it, and a patch test carries more weight, since the risk being managed there is post-inflammatory pigmentation rather than the line itself. In women, collagen loss accelerates in the years after menopause, which is why lines can seem to deepen sharply over a short stretch of the fifties.
What gets mistaken for a wrinkle
Much of what people call a wrinkle is something else, and the difference changes the treatment entirely. Deep folds such as nose to mouth lines form as tissue descends and volume is lost, so relaxing a muscle does nothing for them. Slackness along the jaw is laxity, the process behind jowls. Fine irregularity across the cheek is often surface texture and enlarged pores. Dehydrated or eczematous skin crinkles temporarily and settles within days once the barrier recovers, so treat dryness before judging how lined skin really is. Sleep creases behave the same way, fading over the morning in younger skin and lingering longer as elasticity falls. Most importantly, sun-damaged skin can carry rough scaly patches, actinic keratoses or an early skin cancer inside a crease, and any of those is easy to dismiss as a line.
Causes & Contributing Factors
What Causes Wrinkles?
Wrinkles are caused by skin being folded in the same place, thousands of times a day, while the collagen and elastin that let it spring back are declining. Ultraviolet light does most of the external damage. Smoking, air pollution and the fall in oestrogen at menopause all add to the process, and inherited skin thickness sets where you start from.
Repeated muscle contraction
Facial muscles attach directly to the underside of the skin, so every frown, smile and squint creases it along the same line. Over years that fold becomes an established crease, which is why expression lines trace the pattern of the muscles beneath them.
Collagen and elastin decline
Skin makes progressively less collagen from the mid-twenties onwards, and the elastic fibres already there degrade. With less structural support underneath, skin no longer springs back from folding, so creases that once vanished in seconds start to linger and then stay.
Ultraviolet exposure
Ultraviolet light breaks down dermal collagen and disorganises elastic tissue, a change visible under the microscope as solar elastosis. It is considered the single largest external cause of facial lines, and it keeps working on cloudy days and through window glass.
Smoking and air pollution
Smoking cuts the blood supply to the skin and generates free radicals that degrade collagen, while the repeated pursing of the lips creases the mouth area mechanically. Long-term studies associate particulate air pollution with similar changes in the skin.
Hormonal change
Oestrogen supports skin thickness, collagen content and hydration. The fall at menopause is followed by a comparatively rapid drop in dermal collagen, which is why many women notice lines deepening over a much shorter period than they expected.
Treatments We Offer
How Are Wrinkles Treated at Faciem?
Wrinkle treatment starts by separating lines made by movement from lines held in the structure of the skin, because the two respond to completely different approaches. Skin quality is usually worked on alongside, and folds caused by lost volume are treated as a problem of their own.
01
Anti-Wrinkle Injections
Softening the contraction of a specific muscle eases the line it draws and gives an established crease a chance to settle. Frown lines, forehead lines and lines at the outer corner of the eye are the usual targets, and dosing is planned so your expression still works.
Best for: lines that appear or deepen when you frown, smile or squint

02
Dermal Fillers
Some creases persist because the tissue beneath them has thinned or slipped downwards, and muscle relaxation will not lift them. Restoring support in the right layer softens the crease from underneath instead of smoothing over the top of it.
Best for: static creases and folds left by lost volume
Read about Dermal Fillers
03
Chemical Peels
Peels act on the skin itself, lifting cell turnover and prompting the dermis to rebuild, which improves fine surface lines, crepey texture and uneven tone. This is the part of wrinkle care aimed at sun damage instead of movement.
Best for: fine lines, crepey texture and sun-damaged skin
Read about Chemical Peels
A prescribed medical-grade skincare routine sits underneath all three, and microneedling or platelet-rich plasma may be discussed where collagen stimulation is the priority. Prescription-only medicines are considered in a medical context and never advertised as an offer. Which treatments are used, in what order and how often is worked out face to face with Dr Prasad, and how much change each person sees differs.
When to Seek Medical Advice
When Should Facial Lines Be Checked by a Doctor?
Wrinkles themselves are not a medical problem; the sun-damaged skin they sit in sometimes is. A rough scaly patch, a spot that will not heal, or a sudden change in how one side of the face moves all need a medical appointment, not a cosmetic one.
Worth getting checked
- A rough, scaly or crusted patch that keeps returning to the same spot
- A sore, lump or ulcer within a line that has not healed in four weeks
- Sudden weakness or drooping on one side of the face
- A mole or dark patch within sun-damaged skin that is changing
Conditions that can look similar
- Actinic keratoses, basal cell carcinoma and other sun-related lesions occur in exactly the areas people describe as wrinkled, and they are easy to overlook in skin that already looks weathered. Eczema and simple dryness produce fine crinkling that settles once the barrier recovers. Sudden one-sided facial weakness needs urgent medical attention, not a cosmetic assessment. Anything bleeding, crusting, spreading or refusing to settle should be seen in person.
Self-Care & Prevention
What Home Care Changes About Facial Lines, and What It Cannot
Home care counts for more here than most people expect, because the biggest single cause of facial lines is sun exposure and that one is avoidable. Daily broad-spectrum sunscreen, stopping smoking and a routine matched to your skin all slow the collagen loss that lets creases settle in permanently.
Do
- Apply broad-spectrum SPF 50 every morning, cloudy days included, and reapply every two hours outdoors
- Wear UV400 sunglasses in bright light, which cuts both squinting and ultraviolet exposure around the eyes
- Introduce a retinoid at night two or three evenings a week for the first four weeks, then build up if your skin tolerates it
- Give any new routine a full 12 weeks before judging it, which is roughly one cycle of collagen turnover
Avoid
- Sunbeds altogether, and unprotected sun between 11am and 3pm from March to October
- Smoking, which deepens the lines around the mouth and eyes
- Over-exfoliating — keep acid or scrub exfoliation to two or three times a week at most
- Injectable products bought online, and any treatment given outside a regulated medical setting
This is general guidance and no substitute for individual assessment. Retinoids do not suit everyone, particularly in pregnancy or on sensitive and inflamed skin, and the strength should be matched to your skin at consultation.
Why Patients Choose Faciem
Why Are Lines Assessed Before They Are Treated?
Treating a line without establishing what holds it there is the usual reason a result looks either unchanged or overdone. Movement, lost volume, skin quality and sun damage each produce lines, and each needs something different. Assessment of facial lines is available at Faciem Dermatology & Medical Clinic on Kensington Church St, London, led by Dr Prasad.
- Dynamic and static lines separated before any plan is written
- Skin examined by Dr Prasad, a GMC-registered doctor with 20 years’ experience
- Sun-damaged areas checked for lesions, not simply treated around
- Conservative dosing so that your expression is preserved
Experience & Expertise
Why Wrinkle Treatment Needs a Doctor
Wrinkle treatment goes wrong in predictable ways, and most of them begin with treating a line instead of assessing a face. Reduce the movement of a muscle that is quietly holding the brow up, and the brow drops. Fill a static line in thin, sun-damaged skin, and a ridge appears where a smooth surface was wanted. Treat the forehead of someone whose lines exist because they are compensating for a heavy upper lid, and you change how comfortably they see. Then there is the question of what the line actually is: an actinic keratosis or an early skin cancer sitting inside a crease is not a wrinkle, and weathered skin is precisely where both are found. Dr Prasad brings 20 years’ experience in dermatology and facial aesthetics to that judgement, examining the skin first, watching how the whole face moves, and treating the cause instead of the crease.

Wrinkles FAQs
Your Questions, Answered
Questions people ask about facial lines, from telling one type from another to what genuinely changes them. If yours is not below, put it to Dr Prasad at your consultation.
What causes wrinkles?
Wrinkles are caused by skin being folded repeatedly in the same place while its collagen and elastin decline. Ultraviolet light is the largest external contributor, breaking down the support that lets skin spring back after every expression. Smoking, air pollution, the fall in oestrogen at menopause and inherited skin thickness all influence how quickly lines become established.
What is the difference between dynamic and static wrinkles?
Dynamic wrinkles are visible only while a facial muscle is contracting, so they come and go with frowning, smiling and squinting. Static wrinkles stay visible when the face is completely relaxed, because the crease is now held in the structure of the skin. Most static wrinkles began as dynamic lines, and each type is treated by different means.
How soon can I go back to work or to an event after wrinkle treatment?
Most wrinkle treatment fits around a working week. Anti-wrinkle injections leave small raised bumps at the injection points that settle within about 30 minutes, so people commonly return to work the same afternoon, with any pinpoint bruise lasting a day or two. Dermal fillers can swell for 24 to 48 hours and occasionally bruise for a week. Peels flake for roughly three to seven days. For a wedding or anything photographed, allow two weeks.
How do you get rid of wrinkles?
Which treatment helps depends on the type of line. Lines produced by movement are softened by reducing that movement, which is what anti-wrinkle injections do. Creases held by lost volume need support restored beneath them. Fine surface lines and sun damage respond to resurfacing and prescribed skincare. Most faces need a combination, planned after examination.
Are wrinkles permanent?
Static wrinkles do not resolve by themselves, and the collagen loss beneath them continues, so treatment is maintained over time instead of being a one-off correction. Dynamic wrinkles soften while muscle movement is reduced and return as it recovers. Daily sun protection genuinely slows the rate at which new lines become fixed.
Can I have wrinkle treatment while pregnant or breastfeeding?
Wrinkle treatment is postponed until after pregnancy and breastfeeding. Botulinum toxin and dermal fillers are not given during either, because there is no safety data supporting their use then and nothing about a facial line is urgent. Prescription retinoids are stopped before conception and avoided throughout. What continues is daily broad-spectrum SPF 50, gentle cleansing and a plain moisturiser — and the pigment changes many women notice in pregnancy are worth raising with Dr Prasad in their own right.
What determines the cost of wrinkle treatment?
Cost depends on which lines are being treated, how many areas are involved, whether one approach or a combination is needed, and how often you choose to maintain the result. Prices are published on our anti-wrinkle injection page, and your own figure is confirmed with you in writing at consultation before anything goes ahead.
